Please click on the map to enter your postcode and confirm you live in our practice area.
Inform us if you change your name and address or telephone number. If you move out of the practice area, you will be required to register with a more local practice.
If you wish to register with this practice please speak to the receptionist on duty. They will take you through the registration procedure, requesting that you complete a registration questionnaire, offering you a registration appointment with our healthcare assistant and answering any questions that you may have.
Medical treatment is available from the date of registration. Please contact reception for further information.
If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.
You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.
To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
To register you will need to bring your passport and complete our Temporary Resident Registration form which is also available from reception.
Patients who live outside of the practice area may apply for “Out of Area” registration. This is at the discretion of the doctors as it will be unsuitable for some patients. If you move away and want to remain as an “Out of Area” patient and this is accepted by the doctors you will be asked to reregister. We will be unable to visit you at home if you are not well enough to attend surgery and if this should happen you would need to contact NHS 111 to arrange medical care.
All patients registered at Hillside Practice have a named doctor who has overall responsibility for your care and support. Your registered GP is also your named accountable and your allocated GP. Your allocated GP will be responsible for the provision of your healthcare. If you choose to see another doctor at the surgery you are entirely free to go on doing so exactly as before.
If you are unsure of who is your named Doctor please ask at reception
New patients are allocated a Doctor on Registration.
